Description

We are seeking a compassionate and experienced Registered Nurse (RN) Case Manager to provide skilled, patient-centered care to individuals and families facing end-of-life illness. The Hospice RN Case Manager coordinates the plan of care, completes patient assessments, manages symptoms, administers medications and treatments, provides education and support to patients and caregivers, and collaborates closely with the interdisciplinary hospice team.

The ideal candidate is an organized, compassionate clinician who is comfortable working independently in the field and is committed to providing exceptional end-of-life care with dignity and respect.

Responsibilities include:

Manage an assigned caseload of hospice patients
Complete comprehensive nursing assessments and ongoing reassessments
Develop and coordinate individualized plans of care
Provide symptom management and medication oversight
Educate and support patients, families, and caregivers
Communicate and collaborate with physicians and the interdisciplinary team
Complete timely and accurate clinical documentation
Participate in on-call rotation as required

Requirements

Current Washington State RN license, valid driver’s license, reliable transportation, and strong clinical, communication, and organizational skills. Hospice or home health experience preferred.  Possible company fleet car.
