Physical Therapist (Home Care)

We are seeking a full-time Physical Therapist to join our Visiting Nurse Association (VNA) Home Care team. In this role, you’ll provide personalized, patient-centered therapy across the full continuum of care, helping patients achieve meaningful recovery milestones in the comfort of their homes. This position offers a high level of autonomy, flexible scheduling options, and a supportive environment with structured onboarding, including a comprehensive orientation and mentorship experience.
Responsibilities:
- Perform comprehensive evaluations, re-evaluations, and treatments through discharge using evidence-based practices, establishing accurate assessments, functional goals, and individualized plans of care with appropriate prognosis, frequency, and duration
- Deliver skilled physical therapy interventions, including patient and family education and home exercise programs, while ensuring compassionate, safe, and patient-centered care in accordance with licensing regulations and organizational standards
- Coordinate and direct care activities of licensed and non-licensed personnel (e.g., rehab aides, assistants, volunteers), delegating as appropriate and ensuring adherence to infection control and patient safety practices
- Maintain accurate, timely, and complete documentation, including evaluations, progress notes, discharge summaries, and charge capture with appropriate modifiers and compliance with departmental and organizational standards
- Collaborate and communicate effectively with interdisciplinary teams, patients, families, and referring providers to ensure continuity of care and support program goals and performance initiatives
- Engage in ongoing professional development, adapt to evolving clinical practices and technologies, support student supervision when applicable, and demonstrate professionalism, critical thinking, and flexibility in all aspects of care delivery
Qualifications:
- Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program
- Active Massachusetts Physical Therapist (PT) license required; Basic Life Support (BLS) certification required
- Clinical or rehabilitation experience preferred; orthopedic rehabilitation experience strongly desired
- Demonstrated ability to evaluate, plan, and implement effective patient care using evidence-based practices within regulatory and insurance guidelines
- Strong communication, interpersonal, and leadership skills with the ability to work collaboratively while maintaining independence in a home care setting
- Proficiency with clinical documentation systems and healthcare technology, with a commitment to accuracy, timeliness, and continuous learning
