(Physical Therapist, Physical Therapy Assistant, Occupational Therapist, Speech Therapist)

Employment Type: Independent Contractor
Position Overview
We are seeking licensed Physical Therapists (PT), Physical Therapy Assistants (PTA), Occupational Therapists (OT), and Speech Therapists (ST) to provide skilled therapy services to clients in their homes. Services are delivered in accordance with physician orders, agency policies, and state and federal regulations, with a focus on safety, functional independence, and interdisciplinary collaboration.
Primary Responsibilities (All Roles)
Provide skilled therapy services in the home per physician orders and established plans of care
Evaluate, treat, and educate patients and caregivers within scope of practice
Observe, document, and report patient responses to treatment and changes in condition
Identify and report adverse reactions, contraindications, and safety concerns
Communicate effectively with physicians, caregivers, and interdisciplinary team members
Complete timely and accurate clinical documentation, progress notes, summaries, and discharge reports
Participate in care planning, reassessments, and discharge activities
Role-Specific Highlights
Physical Therapist (PT)
Perform evaluations, OASIS assessments, and develop plans of care
Update care plans based on patient progress and effectiveness of treatment
Instruct patients and families in therapeutic programs and assistive devices
Supervise PTAs monthly and Home Health Aides biweekly when PT is the sole discipline
Physical Therapy Assistant (PTA)
Implement PT plan of care under supervision of the Physical Therapist
Perform therapeutic exercises and functional mobility training
Educate patients and families on exercise programs
Report outcomes and physical findings to the supervising PT
Occupational Therapist (OT)
Evaluate functional abilities and establish OT goals and plans of care
Provide skilled OT services to improve independence in self-care and daily activities
Instruct patients in use of wheelchairs, splints, braces, prosthetics, and adaptive equipment
Educate and counsel patients and families on total OT programs
Communicate regularly with physicians and obtain additional orders as needed
Supervise Occupational Therapy Assistants monthly and Home Health Aides as required
Complete evaluations, progress notes, reassessments, and discharge summaries
Speech Therapist (ST)
Evaluate and treat speech, language, cognitive, and swallowing disorders
Develop and revise speech therapy plans of care in consultation with physicians
Prepare clinical documentation and progress notes
Supervise Home Health Aides biweekly when Speech Therapy is the only discipline involved
Requirements
Qualifications
- Minimum 2 years of experience in respective discipline (home health preferred)
- Strong communication, organizational, and documentation skills
- Ability to work independently and collaboratively
- Computer proficiency and scheduling flexibility
Education, Licensure & Certification
PT: Registered Physical Therapist – State of Florida
PTA: Registered Physical Therapy Assistant – State of Florida
OT: Registered Occupational Therapist – State of Florida
ST: Registered Speech Therapist – State of Florida
All Roles: Current CPR Certification
